Life Threatening Allergies
If your child has a life threatening allergy, please make sure that you have an allergy action plan and any needed emergency medications at school (like Benadryl or an Epi Pen). Action Plans need to be updated at the beginning of every school year. Action plans need to be signed by the parent and health care provider.

Non-Food Allergy Action Plan
(Life threatening allergy to non-food item - bee stings, cold, etc)

Food Allergy Action Plan.
(Life threatening allergy to foods like peanuts, wheat, dairy, shrimp, etc)
