Dakota County Social Services – Children & Family
Transition support services for adolescents as they enter into independent living.
Services can be extended until they are 21 years old if the adolescent is attending high school or an equivalent educational program. The Transition Program works with adolescents who are 16 years or older and are in permanent long-term placement settings. The focus of this program is to help the adolescents prepare for independent living or to assure a smooth transition into an appropriate adult program upon turning 18 years old. DAKOTA COUNTY COLLABORATIVE – what is it?
The Dakota County Collaborative is an integrated Children's Mental Health and Family Services collaborative. Partners include parents, community agencies, schools, and County staff.
The Collaborative allocates federal and State funding to support early intervention and prevention services for children and families in Dakota County. Funds are for programs that prevent family crisis; give more access to existing services; overcome difficulties in the current system; and promote parent leadership and involvement.
